I have 100% parental responsibility of my child after the mother was found to be a risk to the child. Is it possible to make my wife (the child's step mother) a legal guardian so that she can sign school and medical documents etc and make necessary decisions if I am not available? And to protect them both from further legal action from the mother should I die? If so, how do I apply for her to be legal guardian?

Easier to start for a specific power of attorney that only deals with parenting matters for your child.

re: Guardianship. Sorry, I can't help with that question.

Don't bother... The only document that will cause you problems is the passport application... IF you die? lets hope it doesn't happen... But if it did and the child had been in your care for a good length of time, then your partner would have good grounds to seek court orders to maintain custody... The longer you wait to die the more so...
