My house has always been in my name but the ex, when we split, put a caveat on it. The caveat was court ordered to be removed at her expense 3 years ago and it hasn't been done.

I cannot find out how to go about getting it removed.

Any help would be appreciated.

I would attempt to come to some agreement with her that you arrange it and she signs what is necessary and pays for it or something to that effect. if you can't negotiate an outcome, I would get legal advice on enforcing the orders with the court. This will require a further application to court.

If you have a copy of the court order requiring the removal, you should be able to make application to the Registrar of Titles (on a Form 14, General Request) for it to be removed. There's no lodgment fee to remove a lapsed caveat, so I don't think there will be any fees for this.
