I would like to add my spouse to our property title. We have already recently refinanced our mortgage so he is already now included on the mortgage. What is involved with this (paperwork / costs) and is it easy to do?

Go see a lawyer, it is easy and should not cost much. I'm not in NSW and don't know what people charge up there.
I also don’t practice in NSW, but be aware that you will likely need an appraisal or valuation done and then pay stamp duty/transfer proportionately on the interest being transferred.
